Study of Factors
            Various factors could lead to the emergence of hospital associated infections in the health care setting such as increasing the level of patients' acuity, chronically ill patients who accommodates the antibiotic resistant bacteria, and the regular use of wide-scale antibiotic.  Hospital-associated infections could impact on the overall patient outcomes such as mortality rates, the duration of hospital stay, and the overall costs of care (Gagnon, et al., 2012). Placing emphasis on addressing on hospital-associated infection is a critical element of offering quality health care. A targeted model to infections within the hospital settings include prevention strategies, infections within the urinary tracts, and sepsis among others would show the critical information for the nurses taking care for the hospitalized patients.  The identification of such factors causing infections is critical in the improvement of patient safety and quality care in the organization.
Patient Safety Issue
            Hospital acquired infections rates is a critical patient safety issue that should be addressed to improve the quality of health care in the society. In most hospitals, maintaining patient safety is a key priority. Competent  health care professionals  and teams should be able to lower the hospital acquired infections rates, place relevant measures in preventing medical mistakes as well as improving communication between the various stakeholders in the health care facility. Patient safety places focus on the need to avoid bad occurrences and events (Dolansky & Moore, 2013). The improvement of quality in promoting the overall care experience. As a result, hospital acquired infections rates is a serious safety issue for all patients and Omnibus health care facility.

Influence of Nursing Leadership
            The nurse leaders are critical in pushing for changes to reduce the level of hospital acquired infections rates in the health care facility. The nurse leaders have the influence to encourage the nurses to work on reducing the level of medication errors and improving hygiene to reduce the hospital acquired infections rates. Most importantly, the nursing leadership is involved in creation of strategic plans and vision for the health care facility. Such a move would help in improving reduction of infection rates in the long-term period. The leadership functions of nursing usually thrives in complexity and challenging the patient safety concerns through the identification of the new aspects, interaction with other people to create innovative solutions, and creating networks and relationships (Stimpfel, & Aiken, 2013). The nurse leaders could work with entities, organizations, and communities to develop the transformation of the patient safety through enhanced access to quality care in the health care facility.
Effects of Current Policies and Procedures
            Policies and procedures on hospital acquired infections rates in the health care facilities demands for improved health care environment to reduce and eliminate any potential infections on the patients. Health care organizations including Omnibus health care facility have put up in place relevant policies and procedures to prevent hospital acquired infections rates. They include the need for all health care staffs to improve their hand hygiene, the wearing of gloves, need to wear gowns, the protecting of face from potential infectious diseases, the prevention of needle injuries, engaging in regular cleaning, and the importance of respiratory hygiene (Melnyk, Fineout-Overholt, Gallagher-Ford, & Kaplan, 2012). However, it is necessary to implement the right policies and procedures to improve safe hospital environment for all the patients.

Recommendations
            A recommendable evidence-based strategy for dealing with the hospital associated infections is the screening of the safe environment for patients in the hospitals. The screening of the hospital environment will help to ascertain that the health care facilities are able to deliver quality care services. Safe environment for the patient is critical in improving the quality of health care programs and setting. The records office in the health care facility will be able to gather relevant information on all hospital associated infections. The data from the records office will be reliable to understand how to improve safety in the hospitals. The sources of the health care problem can be determined by conducting interviews with the patients as well as the nurses. The interviews would give information to identify the sources of the health problem.  
             The plan to implement the screening of the hospital environment will involve a sequence of steps including the setting the screening parameters, gathering of information on the parameters, analysis of the information, and interpretation of the screening results. The monitoring of the outcomes will be done through hiring inspectors to assist in reducing the hospital associated infections. The quality indicators that will be used to improve patient safety includes improving nursing care, ensuring the provision of medical information, and improving the overall care. These elements of health care will be used to ascertain the level of patient safety and quality care services. Activity monitoring reports and the recording of reviews of care services provision will help in monitoring the outcomes of the efforts to prevent hospital associated infections in the health care facility.
            There is a need to change policies and procedures related to the prevention of hospital associated infections. It would entail recommending changes to prevention and management programs of these infections. Nurse staffs need relevant training on such changes to ensure their efficiency and effectiveness in preventing and managing the hospital associated infections.  The relevant tools that will be used includes health survey tools, online health training programs, and communication action plans to create awareness on the impacts of the hospital associated infections (Hospital Safety Score., n.d). As a result, Omnis health care facility needs to adopt relevant evidence-based strategy to prevent and manage hospital associated infections to improve patient safety in the health care facility.
